Short answer
Use this simple guide before your first claim when you are not sure whether you can bill directly, bill through a group, or work with a biller.
Next step
The three common paths
The words may change by state, but most doulas are trying to sort out one of these paths.
Direct billing
The doula or doula business is set up with the state or payer and can enter or manage claims under its own provider record.
Group billing
A group, agency, collective, clinic, or other enrolled provider may handle the billing path for the doula services.
Biller support
A biller may help enter claims, check payer responses, and follow up, but the payer still decides what provider path is allowed.
What to check before you bill
Use these checks before entering a claim or handing the packet to someone else.
Choose the client state
Direct billing rules depend on the Medicaid program and the payer path for that client.
Check provider enrollment
Find out whether doulas can enroll directly, must contract with managed care, or need another group billing setup.
Check provider identifiers
An NPI, taxonomy, Medicaid provider ID, portal login, or payer profile may be needed, but one identifier does not replace the full setup.
Know who enters the claim
Decide whether you, a group, a collective, a clinic, or a biller will enter the claim and watch the payer response.
Keep records private
Provider IDs, client names, Medicaid IDs, visit notes, claim numbers, and payer messages belong in private records.

Common questions
Can doulas bill Medicaid directly?
Sometimes. It depends on the state, payer, enrollment path, provider identifiers, portal access, and whether the program allows direct billing or uses a group billing path.
Can a doula bill Medicaid without an NPI?
Many billing paths use an NPI or other provider identifier, but the exact requirement depends on the state and payer. An NPI alone does not mean a claim is ready.
Is group billing the same as using a biller?
No. Group billing usually means the claim goes through an enrolled group or organization. A biller may help with claim entry or follow-up, but may not be the enrolled provider.
Can DoulaPaid submit direct Medicaid claims for me?
Not right now. DoulaPaid helps organize setup, claim packets, denials, and payment follow-up. Doulas or their billers still enter claims with the payer.
