- For In-Plan managed care members, submit to the member's MCO; an MCO may require a modifier on the CPT/HCPCS codes.
- The per-pregnancy maximum is $1,500 (3 prenatal + 1 labor/delivery + 3 postnatal); unused prenatal/L&D visits may be billed as postnatal visits up to 7 postnatal total.
- No reimbursement for home births/deliveries. For members under age 21, additional medically necessary visits may be requested via prior authorization.
