Program requirements

  • Program: Oregon Health Plan birth doula services
  • Referral or recommendation: Verify OHP eligibility before service or billing. For EOCCO, a licensed medical professional must identify the THW type and ordered services in the care or service plan; confirm every other CCO's own billing prerequisites separately.
  • Registry or provider setup: OHA enrolls doulas certified by the Traditional Health Worker Program; use NPI taxonomy 374J00000X, provider type 13, specialty 600.
