Program requirements

  • Program: Oklahoma SoonerCare doula services
  • Referral or recommendation: Complete and retain the SoonerCare doula referral form; 8 prenatal/postpartum visits (60-minute minimum, face-to-face or telehealth) plus 1 labor and delivery visit; prior authorization is required for visits beyond the 8-visit package.
  • Registry or provider setup: Birth, postpartum, full-spectrum, or community-based doula certified by an OHCA state-recognized certifying organization; individual NPI; SoonerCare provider contract.
  • Payer path: OHCA SoonerCare provider portal (DDE/837P)
