Privacy requests
Consumer health data requests.
Washington consumers can use this path for access, confirmation, withdrawal, deletion, or appeal requests without creating a new account.
How to submit
Send your request to [email protected] with the subject "Consumer Health Data Request." Include the email address you used with DoulaPaid, the right you want to exercise, and enough non-sensitive context for DoulaPaid to find the relevant account or public interaction.
Do not include patient or client names, dates of birth, Medicaid IDs, claim numbers, service dates tied to a person, visit notes, screenshots with client details, or other private client information in the request email.
Available requests
Access and confirmation
Ask whether DoulaPaid processes consumer health data about you and request access to the data DoulaPaid can authenticate.
Withdrawal
Withdraw consent for collection or sharing where consent is the basis for the activity.
Deletion
Request deletion of consumer health data subject to security, legal, audit, HIPAA, BAA, payer, tax, contract, and backup retention limits.
Appeal
Appeal a denied request by replying to the decision email or emailing [email protected] with Appeal - Consumer Health Data Request in the subject line.
Response timing
DoulaPaid responds to authenticated consumer health data requests without undue delay and no later than 45 days after receipt. If a request reasonably requires more time, DoulaPaid may extend once for another 45 days after telling you the reason within the first 45-day period.
DoulaPaid provides requested records free of charge up to two times per year per consumer. If a request is manifestly unfounded, excessive, or repetitive, DoulaPaid may charge a reasonable fee or decline the request and explain the reason.
Deletion timing
DoulaPaid will process authenticated deletion requests for covered consumer health data unless retention is required or permitted for security, legal, tax, contract, audit, HIPAA, BAA, payer, or records obligations. Consumer health data in archived or backup systems may be deleted on the backup lifecycle after authentication of the request, up to the six-month limit allowed by RCW 19.373.040(3)(c)(iii).
Appeals
If DoulaPaid denies your request, you may appeal by replying to the decision email or emailing [email protected] with the subject "Appeal - Consumer Health Data Request." DoulaPaid responds to appeals within 45 days. If an appeal is denied, DoulaPaid will explain the decision and provide the Washington Attorney General complaint mechanism at atg.wa.gov/file-complaint.
