Basic payment model

Verification pending
Utah's doula codes (T1032 limited to 12 units/180 minutes per pregnancy; T1033 one flat labor support visit) are confirmed, but the Utah fee-schedule dollar rates for the new April 1, 2026 benefit are published only through the PRISM coverage-and-reimbursement lookup and have not been statically verified. Rate calculations and ready-claim checks will be available after official payer rates and codes are verified.
